Replacement Certificates

Replacement Certificates

Requests for a certificate to be replaced/re-issued must be made in writing to the OCN, please follow the process below.

  1. Your centre must complete the Request for a Replacement Certificate Form and send it to OCNSER confirming details of lost or incorrect certificates. The form should provide details of the circumstances leading to the loss or destruction of the certificate. The form is a formal request for a re-issue. In the event of an inaccurate certificate the original must be returned before a replacement can be re-issued.

  2. On receipt of the form OCNSER will authenticate the claim and a replacement certificate will be issued within 35 working days of receipt of request, this will be marked as a reissue. The fee for a replacement certificate is shown in the annual fees and charges available from OCNSER.

  3. When the centre receives the certificate they must sign the Replacement Certificate form which is included in the envelope and return it to OCNSER within five working days.
